Requesting Patient Medical Records and/or Imaging

We offer our patients a secure online process for ordering medical records.

• Gives you the freedom and convenience to order and access your medical records 24 hours a day
• Order and access your medical records from any device (computer, laptop, tablet, phone)
• Processing Timeframes
  • Medical Records - allow 48 hours
  • Imaging – timeframe depends on the delivery method:
    • Secure Imaging Link - allow 48 hours
    • Imaging CD - allow a minimum of 5 business days for CD preparation plus an average of 15 business days for US Postal Service mail delivery to your home address; no guaranteed completion timeframe
• Processing Fees
  • Medical Records - $25 flat fee for all available records
  • Imaging – fee depends on the delivery method:
    • Secure Imaging Link - $15 flat fee for all available imaging
    • Imaging CD - $25 flat fee per requested CD copy for all available imaging
