This ordering site is for use by patients only.

Medical Records

• Processing fee: $25 flat fee per request. There are no refunds for any reason.
• All available Medical Records are provided. You will not be asked to specify the types of Medical Records or dates of service.
• Medical Records will be made available securely online only. We do not fax, email, or mail records.
• Allow 48 hours for your Medical Records request to be completed. You will be texted or emailed when your requested Medical Records are ready to access online (see text samples below).

Imaging (MRIs, X-Rays, etc.)

• For imaging requests, delivery timeframe and fee depend on the delivery method chosen.
• Imaging can be delivered to you either as a Secure Imaging Link or on an Imaging CD. You may select one imaging delivery method per request.
• If you wish to receive BOTH a Secure Imaging Link and an Imaging CD, you must complete two separate imaging requests through this website (one request for each delivery method).
• For whichever delivery method you choose, all available imaging is provided. Note: Imaging reports are not included.
• You will not be asked to specify the types of imaging or dates of service.
• Delivery Option: Secure Imaging Link
  • Processing fee: $15.00 flat fee for a secure link to your imaging. There are no refunds for any reason.
  • You will be texted or emailed a secure link to your imaging within 48 hours of receipt of your order.
  • You may share the secure link with another healthcare provider to view/download your imaging. You can also view/download the images yourself.
• Delivery Option: Imaging CD
  • Processing fee: $25.00 flat fee per Imaging CD copy. There are no refunds for any reason.
  • Your CD will be prepared by the clinic's Radiology office based on their workload and the order your request is received. There is no guaranteed timeframe for Imaging CD preparation. Allow a minimum of 5 business days for CD preparation. You will be texted or emailed when your Imaging CD has been mailed. Please allow an additional average of 15 business days for US Postal Service delivery after you receive the alert that your CD has been mailed. Delivery time required by the US Postal Service varies and is not the responsibility of Carrell Clinic. NOTE: Imaging CDs can only be mailed to your home address.

Physical Therapy Records

• Physical Therapy records are NOT included as part of your Medical Records request.
• To request your Physical Therapy records, contact Fred Weldon at (214) 220-2468 extension 1581 or go to the Physical Therapy and Rehabilitation site ( https://www.carrellclinic.com/on-site-services/physical-therapy-rehabilitation ) and click the "Physical Therapy Records Request" link.

Who Can Order Patient Records Online?

Only patients, or those individuals with the documented legal authority to make healthcare/medical treatment decisions for the patient, can use this ordering site to request medical records and/or Imaging CDs, as described below:

• Patients who have reached the age of majority: 18 years of age or older (NOTE: A parent may not request records on behalf of an adult child, regardless of insurance coverage, residence, etc.).
• Parents/guardians who have the legal authority to access a minor child's records.
• A person who has the documented legal authority to make healthcare/medical treatment decisions for a patient of any age (healthcare conservator, medical/healthcare POA, legal guardian, executor/administrator of estate).

What You Need to Get Started

1. The patient's valid driver's license or other valid government-issued ID.
2. If you are the parent/guardian of a minor patient or have the legal authority to make healthcare/medical treatment decisions for a patient of any age, you will need to upload your valid ID as well.
3. A credit card to make payment.
4. If you have the documented legal authority to make healthcare/medical treatment decisions for a patient, you must also upload official, currently valid documentation of this authority (Durable POA for Healthcare, Medical POA, Advanced Directive for patient and designated caregiver; Death Certificate for deceased patient). NOTE: Financial POA is not sufficient. If you cannot provide both your and the patient's ID as well as documented proof of your legal right to make healthcare/medical treatment decisions for the patient, the patient must place his or her own request.
